The following forms are required to help us here at Suruhåna Massage & Natural Healing work together to serve you better:

If you are a first-time client, please complete the forms below prior to our first session so not to take away from the optimal wellness experience possible; focusing on you and not crucial admin tasks related to your health & well-being.

We welcome you to fill out requested paperwork at the time of service, if needed, please come 5-10 minutes prior to your appointment and forms will be in the lobby for you. If you are late you understand that this time will be deducted from the remainder of the visit.
